1. Eligibility

You must be at least 18 years old, or the age of legal majority in your jurisdiction, to create an account and use the Services unless use is authorized and supervised by a parent, guardian, or legally authorized adult. By using the Services, you represent that you have the legal capacity to enter into these Terms.

2. Description of Services

Anamnesis is a digital platform designed to help users organize, manage, and track personal or family health-related records and information. The Services are provided for informational, organizational, and personal recordkeeping purposes only. They do not provide medical care, diagnosis, treatment, emergency services, or clinical advice. Anamnesis A1 is currently offered as a consumer Personal Health Record application for friends-and-family beta use. It is not currently offered for covered entity, business associate, provider, or regulated clinical workflow use.

3. No Medical Advice

The Services do not constitute medical advice. Nothing in the Services should be interpreted as professional healthcare advice, diagnosis, or treatment. You should always seek the advice of a licensed physician or other qualified healthcare provider with any questions you may have regarding a medical condition. Never disregard professional medical advice because of information stored in or accessed through the Services. If you believe you are experiencing a medical emergency, call 911 or seek emergency medical attention immediately.

4. Account Registration and Security

To access certain features, you may need to create an account. You agree to provide accurate, current, and complete information and to keep it updated. You are responsible for:

• Maintaining the confidentiality of your account credentials
• All activities that occur under your account
• Notifying us promptly of any unauthorized access or suspected security breach

We may suspend or terminate accounts that contain false information or that violate these Terms.

5. User Content

You may submit, upload, store, or manage content through the Services, including health records, notes, documents, images, audio, and profile information, referred to as "User Content." You retain ownership of your User Content. However, by using the Services, you grant us a limited, non-exclusive, revocable license to host, store, process, transmit, and display your User Content solely as necessary to operate, maintain, secure, and improve the Services. You represent and warrant that:

• You own or have the rights necessary to provide the User Content
• Your User Content does not violate any law or third-party rights
• Any information you provide for a child, family member, or dependent is authorized

6. Acceptable Use

You agree not to use the Services to:

• Violate any law or regulation
• Infringe the rights of others
• Upload malicious code, viruses, or harmful material
• Attempt unauthorized access to accounts, systems, or data
• Interfere with the security or performance of the Services
• Misrepresent your identity or authority
• Use the Services for unlawful, abusive, fraudulent, or misleading purposes
• Submit content that is defamatory, obscene, harassing, or otherwise objectionable

We reserve the right to investigate and take action for violations of these Terms.

7. Family Accounts and Dependent Records

If the Services allow you to manage information for children, dependents, family members, or other individuals, you are responsible for ensuring you are legally authorized to do so. You are solely responsible for the accuracy of information entered for other individuals and for managing any permissions or sharing settings.

8. Privacy

Your use of the Services is also governed by our Privacy Policy. By using the Services, you acknowledge that you have reviewed the Privacy Policy.

9. Intellectual Property

The Services, including all software, design, branding, text, graphics, logos, user interface elements, and other content provided by us, are owned by or licensed to Anamnesis, an application of Lee-Stanley Corporation and are protected by applicable intellectual property laws. Except as expressly permitted in these Terms, you may not copy, modify, distribute, sell, reverse engineer, or create derivative works from the Services.

10. Availability and Changes

We may update, modify, suspend, or discontinue any part of the Services at any time, with or without notice. We do not guarantee that the Services will always be available, uninterrupted, secure, or error-free.

11. Third-Party Services and Links

The Services may include integrations with or links to third-party services, tools, or websites. We do not control and are not responsible for third-party content, terms, policies, or practices. Your use of third-party services is governed by those third parties.

12. Fees and Payments

If any part of the Services is offered on a paid basis, you agree to pay all applicable fees as described at the time of purchase. Plans may include a 30-day free trial. After the trial, paid access requires an active subscription. Pricing and billing terms are presented before checkout. Unless otherwise stated:

• After cancellation, access continues until the end of the current billing period
• Fees are non-refundable except as required by law
• Subscription terms renew as disclosed at sign-up
• You are responsible for applicable taxes
• Stripe or another payment processor may be used to process payments

Billing may be tested before full production launch. Billing and subscription enforcement are not yet active during the current beta phase. We may change pricing or billing terms upon notice.

13. Disclaimer of Warranties

THE SERVICES ARE PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS. TO THE MAXIMUM EXTENT PERMITTED BY LAW, ANAMNESIS, AN APPLICATION OF LEE-STANLEY CORPORATION DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ACCURACY, AND AVAILABILITY. WE DO NOT WARRANT THAT THE SERVICES WILL BE ERROR-FREE, SECURE, OR FREE OF HARMFUL COMPONENTS, OR THAT ANY DATA WILL NEVER BE LOST OR CORRUPTED.

14. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, ANAMNESIS, AN APPLICATION OF LEE-STANLEY CORPORATION AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AFFILIATES, PARTNERS, AND SERVICE PROVIDERS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF DATA, REVENUE, PROFITS, GOODWILL, OR BUSINESS OPPORTUNITY, ARISING OUT OF OR RELATING TO YOUR USE OF OR INABILITY TO USE THE SERVICES. TO THE MAXIMUM EXTENT PERMITTED BY LAW, OUR TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATING TO THE SERVICES SHALL NOT EXCEED THE GREATER OF: (A) THE AMOUNT YOU PAID US FOR THE SERVICES IN THE TWELVE MONTHS BEFORE THE CLAIM AROSE, OR (B) ONE HUNDRED U.S. DOLLARS ($100).

15. Indemnification

You agree to defend, indemnify, and hold harmless Anamnesis, an application of Lee-Stanley Corporation and its affiliates, officers, directors, employees, contractors, and agents from and against any claims, liabilities, damages, losses, and expenses, including reasonable attorneys' fees, arising out of or related to:

• Your use of the Services
• Your User Content
• Your violation of these Terms
• Your violation of any rights of another person or entity

16. Termination

We may suspend or terminate your access to the Services at any time, with or without notice, if we believe you violated these Terms, created risk, or used the Services improperly. You may stop using the Services at any time. If you wish to request account deletion, contact us at info@lee-stanley.com. Sections that by their nature should survive termination will survive, including provisions relating to ownership, disclaimers, liability limitations, indemnification, and dispute resolution.

17. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of District of Columbia, without regard to conflict of law principles.

18. Dispute Resolution

Any dispute arising from or relating to these Terms or the Services shall first be addressed through good-faith efforts to resolve the matter informally. If informal resolution is unsuccessful, disputes shall be resolved in the state or federal courts located in Washington, District of Columbia, unless applicable law requires otherwise. You consent to the personal jurisdiction and venue of those courts.

19. Changes to These Terms

We may revise these Terms from time to time. The updated version will be posted with a revised Effective Date. Your continued use of the Services after changes take effect constitutes your acceptance of the revised Terms.

20. SMS Terms and Conditions

Anamnesis is a service of Lee-Stanley, Corp. By opting in to receive SMS messages from Anamnesis, you agree to receive informational text messages related to appointment reminders, appointment confirmations, and service-related notifications.

• Message frequency varies. Message and data rates may apply.
• Consent to receive SMS messages is not a condition of purchase or use of service.
• To opt out, reply STOP at any time. For help, reply HELP or contact info@lee-stanley.com or 301-715-3679.
• No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
• Text messaging originator opt-in data and consent will not be shared with any third parties, except with vendors, aggregators, and providers that support the delivery of our text messages.
